Video Order Form

List of Available Videos

1. You may reserve one of the listed videos on the attached page, which will be sent approximately 5 days in advance of your reservation date. Return to the Grand Lodge Library within 5 days after showing. The Library pays the outgoing postage and Delivery Confirmation (55 cents). You pay the return postage using the same envelope in which you received them. A return label, a Delivery Confirmation label and a new Order Form will be provided. (Delivery Confirmation is a postal service, which allows tracking of a package.)

 
2. If a video is not returned, a late charge of $1.00 per day per video will be assessed until returned. A replacement charge will be billed to you for lost or damaged videos. These charges will be shown as a donation to the Library.
